Grant Report Form

The Community Foundation of the Northern Shenandoah Valley Community Investment Committee and the Board of Directors regularly evaluate our grant programs.  A critical part of the evaluation process is understanding the impact this grant has made for your organization.

Additionally, we like to share information about our grant programs with donors and the community.  Your successes inspire more giving, which ultimately means more grant dollars being returned to our community.  We may use the information and media you submit in presentations, documents and on social media.

Please complete the Grant Report Form below by the deadline for your particular grant.  Submitting a grant report is a requirement for continued participation in our competitive grant programs.  Should your project or program not be complete at the time of submission, please provide an interim report and submit a final report once completed.
