1. Is your organization a nonprofit with the tax-exempt status under section 501(c)(3) of the Internal Revenue Code and not classified as a private foundation.

2. Does your organization serve the City of Winchester, or the Counties of Frederick, Warren or Clarke?
   • Yes
   • No. (If no, you are not eligible.)

    

3. Would requested funds be used for sectarian purposes, i.e., programs that require a particular religious doctrine?
   • No
   • Yes (if yes, you are not eligible)

    

4. Would requested grant funds be used to fund or support political organizations or campaigns?
   • No
   • Yes (if yes, you are not eligible)

    

5. Will you be discriminating based on race, religion, gender, sexual orientation, age, national origin or disability?
   • No
   • Yes (if yes, you are not eligible)
